Garrod and the story of gout
Gout has been around for thousands of years, but our understanding of it changed in 1848 after a simple experiment. That year, an English doctor named Alfred Baring Garrod made an observation that seemed almost accidental, but it changed how people thought about gout. While treating patients with sudden, severe joint pain, Garrod suspected the cause was not temperament or lifestyle, as doctors had believed for centuries, but something in the blood.

To test his idea, he dipped a simple thread into a patient’s blood serum and left it for a few hours. When he checked later, tiny needle-shaped crystals had formed on the thread. These crystals were made of uric acid. This small experiment was a turning point in science. For the first time, there was a clear explanation for gout: too much uric acid in the blood can form crystals in the joints, causing swelling, redness, and intense pain.
Today, this might seem obvious, but in Garrod’s time, it was a big breakthrough. His discovery moved gout away from guesswork and old beliefs to something that could be measured and tested by science.
But the story of gout started long before Garrod.
The ancient Egyptians described a painful condition affecting the big toe. Hippocrates, the famous Greek doctor, called it “the unwalkable disease” because people often could not stand during an attack. The Romans believed that gout was caused by eating too much rich food, drinking too much wine, and indulging in luxury. Over time, this idea became so common that gout was called the “disease of kings.” When rulers like Henry VIII and Louis XIV had it, the reputation grew even stronger.
In those early centuries, treatments were based on trial and error. Doctors tried methods like bloodletting, strict diets, herbal mixtures, and even painful procedures. One plant, the autumn crocus, stood out: it contained a natural chemical later called colchicine, which reduced the inflammation from gout attacks. Colchicine is still used today.
By the 1600s, doctors started to describe gout more accurately. Thomas Sydenham, a respected English doctor who had gout himself, wrote that attacks often happened at night and even the weight of a bedsheet could then feel unbearable. His descriptions helped people see how severe the condition was, but the true cause remained unknown.
The first clue came in the late 1700s, when Swedish chemist Carl Wilhelm Scheele identified uric acid. Scientists now had something to investigate, but it was Garrod who finally proved the link between uric acid and gout. His simple thread experiment showed that the crystals forming in joints were the same as those that could form from uric acid in the blood.
After Garrod’s discovery, treatments slowly became more scientific. In the late 1800s, doctors tried medicines like piperazine to dissolve uric acid, but these early attempts did not work very well. Colchicine stayed the most reliable way to ease pain during attacks.
Real progress happened in the 20th century. Scientists developed medicines that could control uric acid levels in the body, not just treat the pain. The most important of these was allopurinol, introduced in the 1960s. Instead of only reacting to attacks, doctors could now prevent them by lowering uric acid levels. For many people, this turned gout from a recurring nightmare into a manageable condition.
Today, treatment options are even better. Newer drugs like febuxostat and advanced therapies for severe cases give doctors more tools than ever. At the same time, some of the oldest advice still matters: staying hydrated, keeping a healthy weight, and watching certain foods and drinks can help keep uric acid levels under control.
Even with all this progress, old myths about gout remain. Many people still think it is just a disease caused by overeating or drinking too much. In truth, gout is influenced by genetics, kidney function, metabolism, and other health conditions. Lifestyle matters, but it is only one part of a much bigger picture.
Looking back, the history of gout is a lot like the history of medicine itself. It starts with ancient guesses, goes through centuries of observation, and reaches a turning point with Garrod’s simple thread experiment. After that, it moves into the modern age of chemistry, lab testing, and targeted treatment. What was once seen as a mysterious punishment for indulgence _ one that was to be “treated” with torture _is now understood as a well-defined medical condition, thanks to centuries of scientific curiosity, observation, and progress.
